Immunization and TB Screening

For new students, your initial TB screening should be completed within 90 days of your program start. Professional, continuing students are required to have annual TB testing. Reach out to your primary care provider to request a Quantiferon lab test. Results should be uploaded into immunization record section in myhealthrecord.ucsf.edu.

You may obtain immunization records* at any time. The fastest, easiest way is to log on to MyHealthRecord, click on the “Immunization” section and print your own copy. If you are unable to access the records online, you may visit SMHW in person or fax the Medical Release form to SMHW (415) 476-6137. There is no fee for printing immunization records.
